The initial appointment is a pre-scheduled, in-person assessment appointment that takes 1.5-2 hours. At that time, you will meet with a qualified counselor to discuss your specific situation and needs related to
substance abuse and mental health challenges .
Please bring a valid picture ID, your insurance card, and any current prescriptions or over-the counter medication you are taking.
If you are being referred by another provider, court liaison, probation, or attorney, please bring their contact information
and paperwork they have given you.
For minors, parents will need to bring the child's birth certificate, custody/guardianship paperwork, or other papers confirming custody.
All assessment fees are due at the time of the assessment to include unmet deductibles, copay, or cost-share.
At the time of the assessment, the counselor will make a preliminary recommendation for treatment and/or clinical support appropriate for your needs and based on the American Society of Addiction Medicine (ASAM) placement criteria guide and other pertinent clinical information.
Final treatment recommendations are confirmed within 72 hours of the initial assessment for all non-emergent cases. Per client request and with written consent, an assessment summary can be sent to any 3rd party for which assessment notification is needed.
After the initial assessment appointment, admission and options for ongoing clinical services at Encompass will be discussed with each client individually.
